Palpatine's deformed appearance was part of his plan to turn people against the Jedi, so I think he may have let himself become deformed by the fight with Windu. Just as he made it look like Mace was all over him, so Anakin would intervene to 'save' the Chancellor.
In the novelization for RotS, Palpatine states the "The mask is gone now." He says it's a mask himself. Since novelizations of the movie are considered canon, there you go...

The Emperor was deformed when he used the power and drained himself and shed the human skin and revealed his true identity.
Because Mace reflected it back to Palpatine wit his lightsaber. The lightning plus the power of the kyber crystal deformed him

It's the sustained use of Force lightning that deformed Sidious. Short bursts, like those used by Dooku in Episode II (and by Sidious early on the encounter with the Jedi) have little effect. Getting hit by it doesn't cause deformities at all, just intense pain.
It is the dangers of using force lightning. This is something every sith who uses force lightning has to come to terms with. The longer it is used the greater the danger. Short bursts are relatively safe while prolonged use can kill the force user.
